Conduct outreach and engage with community members to assess their health needs and provide information on available resources and services. Build and maintain strong relationships with community organizations, healthcare providers, and other stakeholders to promote collaboration and enhance access to health services. Educate individuals and groups about healthy behaviors, disease prevention, and management of chronic conditions. Advocate for and support individuals in navigating the healthcare system and accessing appropriate services. Conduct home visits and provide one-on-one support to individuals with complex health needs. Collaborate with interdisciplinary teams to develop and implement community health programs and initiatives. Collect and maintain accurate data on health outcomes and program effectiveness. Identify and address social determinants of health that may impact individual and community health. Attend community events and meetings to raise awareness about health issues and promote health education.
