This individual will be expected to monitor a specific group of patient s care and support them in achieving short/long term health goals. The Care Manager will coordinate with providers to ensure patients needs are met. A working knowledge of the clinical field is desired for this position. This is a Hybrid position, please note first week of training will take place in Scottsdale, AZ. Care Manager Responsibilities  Manage a roster of patients who are enrolled in the Hybrid Patient Monitoring program (RPM).  Act as a direct liaison between the patient and the office/provider.  Scheduling and confirming appointments in office for the patient.  Have a full understanding of each patient s individual care plan and making sure they are following it.  Record patient data daily and alert healthcare providers of abnormal reports.
