Key Responsibilities • Review and analyze incoming patient data, including vital signs, symptoms, and other relevant health indicators, to identify any critical or out-of-range readings. • Promptly address any critical or out-of-range readings and escalate as necessary according to established protocols. • Monitor patients' vital signs and health data remotely using specialized monitoring equipment and software. • Communicate with patients via phone to discuss their health status, provide guidance, and answer questions. • Explain medical terminology and procedures in an understandable manner, ensuring patients are informed and engaged in their healthcare management. • Address patient concerns and questions promptly, providing reassurance and guidance where necessary. • Manage a significant volume of patient calls within established timelines. • Work towards achieving personal and team performance metrics. • Foster a collaborative and positive work environment. • Maintain the Nsight culture of positivity. • Participate in cross-training and other LVN duties as assigned.
