RN Nurse Manager Rehab and Long-Term Care

LifeBridge Health is seeking an experienced RN Nurse Manager for Rehab and Long-Term Care to provide clinical and operational leadership within our Healthcare & Medical Services department. In this role, you will oversee nursing care for patients and residents requiring rehabilitation and long-term support, ensuring that services are delivered with compassion, excellence, and adherence to best practices and regulatory standards. The RN Nurse Manager is responsible for supervising and mentoring a team of RNs, LPNs, and nursing assistants, fostering a collaborative and supportive environment that reflects LifeBridge Health’s culture of innovation, teamwork, and community engagement.

You will coordinate daily operations on the unit, including staffing, scheduling, workload management, and oversight of clinical care, while promoting a safe, respectful, and patient-centered atmosphere for individuals with complex medical and functional needs. Working closely with an interdisciplinary team that may include physicians, therapists, social workers, case managers, and other healthcare professionals, you will guide care planning, transitions of care, and discharge processes to optimize outcomes and promote functional recovery and quality of life. You will lead quality improvement and patient safety initiatives specific to rehab and long-term care, focusing on areas such as falls reduction, pressure injury prevention, medication safety, infection prevention, and effective pain and symptom management.

The RN Nurse Manager ensures compliance with all applicable regulations and accreditation requirements, including documentation standards, infection control, and safety policies relevant to rehabilitation and long-term care settings. You will use data, clinical evidence, and feedback from patients, families, and staff to drive continuous improvement in both clinical outcomes and patient experience. In addition to clinical leadership, the RN Nurse Manager will participate in hiring, onboarding, training, and evaluating nursing staff, supporting ongoing professional development and career growth.

You will help manage unit resources and contribute to budget stewardship by aligning staffing and supplies with patient needs and organizational priorities, while maintaining high standards of care.
